Description	Two leaves, typewritten on the front sides. The document is in good physical condition. There are paper clip rust stains at the upper left corners of both leaves. There are staple holes at the upper left corners of both leaves. There is a small paper clip tear at the upper left corner of the first leaf. There is an "8 handwritten in ink at the upper right corner of the first leaf, likely for classification purposes. The document appears to be a carbon duplicate.
