Description	Five leaves of lined paper, handwritten in pencil on the front sides of pages one through four. Page five is handwritten in pencil on both sides. The document is in good physical condition. The document was lamianted. There are several lamination-related crease throughout the document. The upper left corner of the third leaf was folded into the laminate. The document was folded vertically into halves, possibly for mailing. There are musical notations beneath the document title. The informant citation is located on the reverse side of the fifth leaf.
