Description	Four leaves of lined paper, handwritten in pencil on the front sides. The document is in good physical condition. The document was laminated, with the leaves back-to-back. There is lamination-related creasing throughout the document. There are staple holes and related rust stains at the center upper edge of each leaf. There is staining of unknown origin throughout the document. There are two minor tear holes at the lower right edge of the fourth leaf. The document was folded horizontally into thirds, possibly for mailing. There are handwritten page numbers at the center top edge of each leaf.
