Description	One leaf, typewritten on the front side. The document is in good physical condition. There are staple holes at the upper left corner. There is one hole at the upper right corner. There are paper clip rust stains at the upper left corner. There are minor tears along the right edge of the document. Both a "1 and a "9 are handwritten in ink at the top right corner, likely for classification purposes. A "2 was handwritten in pencil over the "9. "
