Description	One leaf with preprinted staff lines, handwritten in black ink on the front side. The document appears to be from a pad perforated along the top edge. The document is in good physical condition. The document was laminated. There is a moderate tear at the upper left edge. There is a paper clip rust stain at the center top edge. There is a check mark handwritten at the lower right corner. There is a note handwritten in pencil at the upper right corner.
