Description	One leaf, handwritten in pencil on the front. The document is in good physical condition. There are two loose leaf binding holes at the right edge. The document was laminated. The bottom left corner was folded into the laminate. There is a minor tear at the upper right corner. There are two minor tears along the top edge. There are moderate tear holes at the center of the document. The document was folded horizontally and vertically into sixths.
